A 48-year-old male presented to the emergency department of his primary care clinic with sudden onset of right heart failure and left ventricular hypertrophy, presenting with persistent hypotension and hypotension and severe hypotension with a right atrial (R AV) diameter of 6 mm. Tadalafil was prescribed for the treatment of pulmonary hypertension (PAH), a disorder that has been associated with a high incidence of pulmonary arterial hypertension (PAH) and left ventricular hypertrophy (LVH). A pulmonary artery wedge pressure (PAP) of 20 mmHg was present. PAP was measured with a digital x-ray. Hemodynamic studies were performed, and PAH was diagnosed with pulmonary arterial hypertension (PAH) with pulmonary capillary wedge pressure (PCCVP). PAH was treated with tadalafil (Cialis®) 20 mg three times daily, starting at 36 hours, then 40 mg daily. Tadalafil was stopped on day 40, and PAP was reduced to 10 mmHg by the same protocol. On day 56, the patient was referred for the evaluation of left ventricular (LV) dysfunction in the right heart catheterization (RHC). Left ventricular (LV) systolic function (left ventricular ejection fraction) decreased to 9% on day 37. Right ventricular (RV) systolic function (left ventricular end diastolic pressure (LVEDP)) increased to 25 mmHg on day 56. Right ventricular systolic function (systolic and diastolic pressures) improved to 51 mmHg on day 58. Patient was started on tadalafil 20 mg daily and maintained on tadalafil 20 mg and tadalafil 20 mg once daily. Tadalafil 20 mg and tadalafil 20 mg were well-tolerated with a favorable effect on PAP, RVEDP, and RVEDP, with no adverse effects on LVH or RV dilation. Tadalafil 20 mg was well tolerated with a few cases of severe adverse effects. Tadalafil 20 mg was discontinued on day 56.
Erectile dysfunction is the most common cause of pulmonary arterial hypertension (PAH), affecting 1-4% of the general population [1]. PAH is characterized by increased cardiac work-ups, including left ventricular (LV) systolic dysfunction and diastolic dysfunction, which results in impaired quality of life [2,3]. There have been cases of sudden death and cardiac arrest in this population. Pulmonary arterial hypertension (PAH) can occur at any age [4]. PAH has been associated with poor outcomes and is often treated with specific therapies such as antihypertensive drugs [5]. However, the exact cause of PAH remains unknown, but it is believed that it is due to several factors including systemic vascular disease and genetic factors [6]. Tadalafil 20 mg and tadalafil 20 mg were not effective in controlling pulmonary arterial hypertension (PAH) with PCCVP [7]. Tadalafil 20 mg should not be used in patients who do not respond to specific therapies or who have a history of hypertension. Tadalafil 20 mg should be used with caution in patients with a history of hypertension who are at increased risk of sudden cardiac death [8].
A meta-analysis of randomized controlled trials (RCTs) found that there were no differences in outcomes in patients with PAH who received tadalafil 20 mg compared with placebo [9]. However, there was a reduction in mortality with tadalafil 20 mg compared with placebo in the RCTs (23.6% vs. 21.4%, respectively). There were also fewer cases of cardiac arrest with tadalafil 20 mg compared with placebo [10]. There were no significant differences in outcomes in the tadalafil 20 mg arm of the meta-analysis [11].
A systematic review of RCTs and meta-analyses found a statistically significant reduction in adverse events with tadalafil 20 mg compared with placebo in the treatment of pulmonary arterial hypertension (PAH) [12].
1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).
2.Alpha-blockers (e.g., prazosin, terazosin, tamsulosin): Both Cialis and alpha-blockers relax blood vessels, which can lead to excessive blood pressure reduction when used together. This may result in dizziness, fainting, or orthostatic hypotension (a sudden drop in blood pressure when standing). If needed, your doctor may adjust the dosage or recommend spacing the medications.
